    Upon arrival, please inform your server or manager of your chosen Party Package. To ensure a smooth experience, at least one member of your party should arrive within 15 minutes of your reservation. Late arrivals may result in releasing the table. No refunds will be given. You may bring freestanding or floating decorations, but confetti, glitter, and similar items are prohibited and may incur cleanup fees. Pricing excludes tax and gratuity. Per Maricopa County Health Codes, outside food and drinks are prohibited. A $100 deposit is required to secure your reservation, non-refundable and applied to your final bill. Deposits are non-refundable. The Forum may cancel reservations for event buyouts, with full deposit refunds processed.

    Cute little hidden gem! First time attending and they had a blues band. Absolutely loved it. Will be going back soon.

    Food was mid, service was scatter brained and lacking. Dietary restrictions were communicated but were not actually enforced. Cheese served to a vegan diner which was communicated from the beginning. Overall regret leaving the tip amount I left. On the positive side, the band was amazing.

    Very satisfied with the service, was there seeing Guy pannachio , singing Frank Sinatra song , very pleasant with the service, we will came back again

    Thursday were a little more lively last October. This was a very mellow night. The Forum is a huge place and it was a little empty tonight. It would be nice if the drinks were offered as happy hour.

    The waiter was great. We love the burgers. Drinks were delicious, too. We enjoyed the band tonight. Slingshot.

    I loved everything about my night except the waitress service I think when it comes to the vip room there should be more than one server we waited soooo long for service and the glasses piled up on the tables when we finally got drinks and no one ever came to clean them off we didn’t even get to order food !!!! Now that was poor service! But everything else was great!!!!

    In the parking lot there was a stripper van and we were very uncomfortable with the ladies in the van.

    Had a fantastic time the band was amazing! The food was awesome. The only downside the whole night was the waitress was very slow and then they hid a 20% tip into the final bill without telling anybody, that got my wife really upset. However, I would still very much recommend this establishment.
